Welcome to CSMS Lady Cubs Athletics. Here you will find information about the sports offered at CSMS and general requirements for athletics.

Attention all parents of incoming 7th graders:
In order to participate in athletics, all athletes must have a UIL Physical form completed by a doctor. The physical MUST be on the UIL form. You may pick one up from the school office, coaches’ office, or print one out from the UIL Forms link on the left column. The UIL Forms link also has the UIL Rules Acknowledgement and Emergency Travel Card forms. All of these forms must be on file to be in athletics.


8th Grade Parents and Athletes:
Physicals are good for two years, so if you participated in athletics last year, then your physical is still good. If you are an 8th grader new to athletics, you will need to have a physical on file. Every 8th grader must complete all other UIL Forms (UIL Rules Acknowledgement and Emergency Travel Card). You may pick these up from the school office, coaches’ office, or print one out from the UIL Forms link on the left column.


NOTE:
Physicals MUST be on the UIL Physical Form. Any physicals on other forms will not be accepted. All athletes must have a physical on file. If they do not, they are not allowed to participate in the athletics class. Failure to have a completed physical may result in removal from athletics.


        We expect top notch behavior   .
Treat all teachers, coaches, etc. with respect.
Avoid getting in trouble both in and out of school.
Try your best both on the court and in the classroom.
