The UIL sets the rules and requirements for all athletic physicals.
Physical forms are required for ALL athletes of CSISD.
Physicals must be completed on Current UIL physical forms.
Physicals must be completed before any workouts/tryouts of that academic season.
CSISD offers $10 physicals every spring for athletes of CSISD.
Incoming 7th, 9th and 11th graders
The UIL requires these grade athletes to have:
A NEW physical on file
An updated medical history
An updated emergency card
A yearly steroid form agreement
A yearly acknowledgement of rules forms
Incoming 8th, 10th and 12th graders
